The supporting material should include:

A maximum limit of 10 pages for nomination narratives. We suggest using Times New Roman 12 point, 1.5 line spacing and margins equal to or greater than .75 inches on all sides
Documentation of an individual's or team’s professional research achievements in narrative form, including significant publications or products using significant research papers, presentations at professional meetings, and other information, detailing the specific contributions
Length and continuity of Society membership
Related Awards received from the Society or other organizations
The nominees Professional Curriculum Vitae must accompany the Award Nomination Questionnaire (maximum limit of 20 pages – we suggest using Times New Roman 12 point, 1.5 line spacing and margins equal to or greater than .75 inches on all sides)

The Award Nomination Questionnaire and supporting material may be emailed to: awards@swcs.org. If emailing, you may fax the signed petition to 515-289-1227. You may also submit the Award nomination by U.S. Postal Service (1 copy only) to: SWCS-Awards, 945 SW Ankeny Rd., Ankeny, IA 50023. All nominations must be received by February 12. All forms of submission will receive a conformation of receipt. Late nominations will not be considered.
